Client Account Change Request Form

Please use the Client Account Change Request to notify us or request changes.

You may use the form to make one or more of the following changes.

Update account information

Add Users

Delete Users

Change user privileges

Change the Prime Contact

Close an account

Instructions:

Provide your current (old) name, address, and telephone numbers and the new name, address and telephone numbers.

Instructions:

To add new users to your account, enter the names of the users under "Add New Users".

By default, new users will be assigned Basic administrative privileges and Search and Register Personal Property Registry capabilities. If you wish to create new users with other access privileges, list the access privileges under "Comments / Special Instructions" or use the Client Application Addendum form. You may want to consult a list of ACOL PPRS Access Privileges

If you wish to add more users than the space provides, attach an additional page listing the names new users to be added. Please indicate under "Comments/Special Instructions" that there is a second page of information attached.

Instructions:

To delete existing users, enter the name and user ID for each user.

If you wish to delete more users than the space provides, attach an additional page listing the names of users to be deleted. Please indicate under "Comments/Special Instructions" that there is a second page of information attached.

Instructions:

To change the access privileges for a user, enter the person's name, user ID and desired access privileges under "Comments / Special Instructions".

To change the primary administrative contact person for your account, enter the user's name and user ID under "Change Primary Contact To".

If you wish to change the primary administrative contact person to a new user that you are creating with this request, please write "New user" after "User ID".

To close your account select that option under "Designate Type of Change" near the top of the form.

Before closing your account, please read more about closing your account.

It is also possible to suspend your account -- temporarily preventing any use of the account -- without permanently closing it. If you wish to only suspend your account, do the following:

Select the option to close your account.
Under "Comments / Special Instructions" at the bottom of the form, enter an instruction to suspend, not close, the account.

General Instructions:

Identify your account

At the top of the form, provide your ACOL client account number (Client ID) and contact numbers. Your telepone contact number is important in case we have questions in order to complete your requested change.

Sign the form

An authorized signature is required in order for us to complete the request.

Designate the type of change

Select the type of account change that you are requesting. You may perform more than one type of change in a single form.

Send the form to us

Please ensure that an authorized representative has signed the form before you send it to us. Send the form by email to ACOLBusinessOffice@Unisys.com or mail it to:
