ACOL Client Forms

ACOL offers its client forms (included in the Welcome Package) in Portable Document Format (PDF) for your convenience.

The Adobe(R) Acrobat(R) Reader allows you to view, navigate, and print PDF files across all major computing platforms. You can download it from the Adobe web site free of charge.

Please use the ACOL Client Account Change Request to notify the Client Support Centre whenever there are changes to your client account. It is important to keep your account information current. Fax the form to 1-902-422-1675 or mail it to the Client Support Centre.
